A Third of the World’s Population Has More Than 5 Health Problems: Including Low Back Pain

With a growing world population and a growing proportion of elderly, more people are living in suboptimal health around the world. A third of the world’s population experienced more than 5 ailments in 2013, according to a study published in The Lancet. In comparison, only 4.3% of the world’s population had no health problems. In addition, the Global Burden of Disease Study 2013 collaborators found that from 1990 to 2013 the proportion of years of healthy life lost due to illness rose from 21% to 31%. Rates of disability are declining slower than death rates. “The fact that mortality is declining faster than non-fatal disease and injury prevalence is further evidence of the importance of paying attention to the rising health loss from these leading causes of disability, and not simply focusing on reducing mortality,” Theo Vos, lead author and professor of global health at the Institute of Health Metrics and Evaluation, University of Washington, said in a statement. The researchers used 35,620 sources of data from 188 countries between 1990 and 2013 to estimate the overall burden on healths systems from 301 acute and chronic diseases and injuries and 2337 health consequences from these diseases. During the 23-year period studied, the leading causes of health loss did not change. Low back pain, depression, iron-deficiency anaemia, neck pain, and age-related hearing loss accounted for the largest overall health loss in terms of years lived with disability. Low back pain and depression were among the top 10 greatest contributors to disability in every country studied. These 2 diseases caused more health loss than diabetes, chronic obstructive pulmonary disease, and asthma combined. However, over the time studied there had been a sharp increase in the health loss associated with diabetes (increase of 136%). Other diseases with startling increases were medication overuse headache (120%), Alzheimer’s disease (92%), and osteoarthritis (75%). “Large, preventable causes of health loss, particularly serious musculoskeletal disorders and mental and behavioural disorders, have not received the attention that they deserve,” Dr Vos said. “Addressing these issues will require a shift in health priorities around the world, not just to keep people alive into old age, but also to keep them healthy.”

Change in lumbar lordosis during prone lying knee flexion test in subjects with and without low back pain

Prone lying knee flexion (PLKF) is one of the clinical tests used for assessment of the lumbo-pelvic movement pattern. Considerable increase in lumbar lordosis during this test has been considered as impairment of movement patterns in lumbar-pelvic region.

However, no study has directly evaluated the change in lordosis during active PLKF test in subjects with low back pain (LBP). The purpose of this study was to investigate the change of lumbar lordosis in PLKF test in subjects with and without LBP.

Methods: A convenience sample of 80 subjects participated in the study.

Subjects were categorized into two groups: those with chronic non-specific LBP (N = 40, mean age: 40.84 ±17.59) and with no history of LBP (N = 40, mean age: 23.57 ±10.61). Lumbar lordosis was measured with flexible ruler, first in prone position and then on active PKF test in both subjects with and without LBP.

Data was analyzed by using statistical methods such as, independent t-test and paired t-test.

Results: There were statistically significant differences in lumbar lordosis between prone position and after active PLKF in both subjects with and without LBP (P <0.0001). The amount of change in lordosis during PLKF test was not significant between the two groups (P = 0.65).

However these changes were greater among patients with LBP.

Conclusion: Increase in lumbar lordosis during this test may be due to excessive flexibility of movement of the lumbar spine in the direction of extension and abnormal movement patterns in the individuals with LBP.

Rick Greenwood thought surgery would relieve his back pain. Instead it left him paralyzed. He developed a blood clot after having a battery-powered stimulator implanted next to his spinal cord.

“My whole focus was getting off pain meds, ’cause it was ruining my life. But did we check it maybe as thorough as we needed to on the side effects? No,” he said.

Rick and his wife Debbie urge others to ask tough questions before surgery.

“Questions would be more about specifically how many surgeries have you done like this? What are the guidelines out there for physicians? What happens if something goes wrong?” she said.

“I think people need to be mindful of physicians and surgeons who are well-trained, and to investigate their training,” said Dr. Stephen Tolhurst.

Other questions to ask: Is my surgeon board-certified and fellowship-trained? How long has my surgeon been performing this procedure? What is the success rate? Will anyone assist my surgeon and if so, how are they trained? What are the risks? Are there other non-surgical options?

“I’m hoping that this will help people feel more comfortable in researching and asking the questions. You just gotta hang in there,” said Rick.

He hopes asking the right questions will prevent others from suffering like he did.

Time Magazine Releases Cover Story on Pain Killers

The June 15 issue of Time Magazine is out with a cover story on Pain Killers that says America’s addiction to them is the worst addiction crisis the country has ever seen.

Calling it a national epidemic, it says “9.4 million Americans take opioids for long-term pain and 2.1 million are estimated by the NIH (National Institutes of Health) to be hooked.”

The article written by Massimo Calabresi calls the crisis a “tragic combination of good intentions, criminal deception and feckless oversight to turn America’s desire to relieve its pain into such widespread suffering.”

It recounts how the FDA approved ever more powerful drugs for long-term use without enough data, the pharmaceutical companies marketed aggressively, and doctors wrote the prescriptions too freely.

The article doesn’t speak much to the issue of chronic pain, for which many prescriptions are written, other than to say 100-million people suffer from it and a quarter of them says it is severe enough to limit their quality of life, according to the Institutes of Medicine.

The National Pain Report ran a story in April asking if the media are missing the point on the pain medication dispute and cited two pain leaders who agreed that prescription drug abuse is a problem, but that pain patients who need the medication and use it responsibly needs to be remembered in the discussion.

The Time article really concentrated on the addiction and the lack of science it says exists. Calabresi writes:

“Part of the problem, according to the NIH, is that doctors have no scientific certainty over when and whether it’s safe to use opioids to treat long-term pain. “There is insufficient evidence for every clinical decision that a provider needs to make regarding use of opioids for chronic pain,” a NIH panel on opioids concluded earlier this year. The American Academy of Neurology last year concluded that the risks of long-term opioid treatment for headaches and chronic low-back pain likely outweigh the benefits.”

The lack of science both retrospectively and going forward was cited in the story.

“There were no reliable studies proving opioids worked safely against chronic pain, because it would be unethical to require pain patients in a control group to go months on end without medication. “It’s not practical for us to require people to go for a year on a placebo,” says Janet Woodcock, head of the FDA’s Center for Drug Evaluation and Research”

The Time article also says the issue is starting to percolate in the nascent 2016 presidential campaign. Hilary Clinton is talking about the “quiet epidemic” and Carly Fiorina, whose late daughter struggled with painkillers, has call for decriminalizing drug addiction.
